A monitoring inspection was conducted on 3/20/2023 with the regional director and center staff. There were 62 children present, ranging in ages from 5 years to12 years, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Staff records were shared electronically on 3/21/2023. A total of 6 child records and 3 staff records were reviewed. The children were making paper kites, playing outdoors, and playing a group game in the gym.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Based on review of six children's records, the center did not obtain all required information for two children's records. Evidence:
1. The record of child # 3 did not have the name and phone number of the child's physician.
2. The record of child # 5 did not have the name and phone number of the child's physician or the name and phone number of each parent's employment.

Plan of Correction: Center staff will request the information from the parents.
