Comments:
An unannounced monitoring inspection was conducted 3/8/2023 from 3:30pm-5:30pm. There were 34 children directly supervised by 8 staff and all groups were in ratio. The physical plant, outdoor playground area, programming, menus, 3 staff records, 6 children?s records, 1 medication, emergency drills, injury reports, emergency supplies, and policies were inspected. Children were observed doing homework, playing indoor hockey, playing outside, reading books, and eating snack which consisted of mixed fruit, cole slaw, fish patty, and bun. Positive interactions between children and staff were observed during this inspection. There was an adequate number of CPR/First Aid, Daily Health Observation, and MAT trained staff on site at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22.1-289.035-B-4
Description: Based on record review, the center did not obtain the results of an out of state criminal history background check prior to the first day of employment for all staff who have resided outside the state of Virginia within the last 5 years.

Evidence: Staff #1 (Date of Hire: 11/22/2022) has resided in the state of Alabama within the last 5 years and did not have an Alabama criminal history background check search result on file at the time of inspection.

Plan of Correction: These will be completed prior to new employees starting work.

Currently in communication with Alabama state and Fieldprint to determine how to obtain.

Standard #: 8VAC20-770-60-C-2
Description: Based on record review, the center did not obtain the results of a Virginia Child Abuse and Neglect Central Registry for all staff within 30 days of employment.

Evidence: Staff #2 (Date of Hire: 9/28/2022) and Staff #3 (Date of Hire: 6/20/2022) did not have the results of of a Virginia Child Abuse and Neglect Central Registry search on file at the time of inspection.

Plan of Correction: Moving forward CPS checks will be submitted online within the first week of employment. These were submitted online 4/3/23.

Standard #: 8VAC20-780-70
Description: Based on record review, the center did not ensure there was documentation on file for all staff that two or more references as to character and reputation as well as competency were checked before employment.

Evidence: Staff #1 (Date of Hire: 11/22/2022) and Staff #3 (Date of Hire: 6/20/2022) were both present at the time of inspection and did not have reference checks on file.

Plan of Correction: Calls made to references to complete checks. Moving forward calls will be made prior to hiring.

Standard #: 8VAC20-780-240-A
Description: Based on record review and staff interview, the center did not ensure that all staff complete the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.

Evidence: Staff #1 (Date of Hire: 11/22/2022) has not completed the 10 hour Virginia Department of Education-sponsored orientation course.

Plan of Correction: Staff has been given time to complete training.

Standard #: 8VAC20-780-260-A
Description: Based on record review and staff interview, the center has not had an annual fire inspection.

Evidence: The most recent fire inspection report on file was dated 2/1/2022.

Plan of Correction: Called Fire Department for inspection.

Standard #: 8VAC20-780-340-F
Description: Based on observation, the center did not ensure that children under age 10 years of age were always within actual sight and sound supervision.

Evidence: Multiple groups of children under age 10 were observed going up and down the stairs between the first and second floor throughout the inspection without sight and sound supervision of staff.

Plan of Correction: Members under the age of 10 will be escorted throughout the building by a staff member.

Standard #: 8VAC20-780-510-P
Description: Based on review of medications and documentation, the center did not ensure that medications are returned to parents within 14 days after an authorization for a medication expires or the parent must renew the authorization.

Evidence: Child #7 had an emergency medication on site with a physician authorization that expired 11/6/2022.

Plan of Correction: All medications will be returned to parents within 14 days of expiring.

Medication was returned to parent.
