Comments:
A non-mandated monitoring inspection was conducted on 03/23/2023 from 9:20 A.M.-12:45 P.M. There were twelve children present, ranging in ages from 9 months to five years of age, with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on review of children records, the center failed to ensure that all required information were in children records.

Evidence:
1. Child #1's record was missing the date of enrollment and physician's phone number.
2. Child #2's record was missing the date of enrollment, physician's phone number, and a complete emergency address for one of the emergency contacts.
3. Child #3's record was missing the date of enrollment, the name, home address, and phone number of each person having custody of the child, physician's phone number, and emergency address for one of the emergency contacts.
4. Child #4's record was missing the date of enrollment, the name, home address, and phone number of each person having custody of the child, and the name, address, phone number of two designated people to call in an emergency if a parent cannot be reached.
5. Child #5's record was missing the date of enrollment, and the name home address, and home phone number of each person who has custody of the child.

Plan of Correction: Administration will meet with parents and obtain all missing information. In the future all new enrollments paperwork will be reviewed to ensure it is complete and accurate.

Standard #: 8VAC20-780-90--A
Description: Based on review of children records, the center failed to ensure that all written agreements between the parent and the center shall be in each child's record by the first day of the child's attendance.

Evidence:
There were five children records reviewed. The date of attendance for all five children was 01/31/2023. The following agreements were not in the children's records; an authorization for emergency medical care should an emergency occur when the parent cannot be located immediately, and a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease.

Plan of Correction: Administration will meet with parents and review the agreements and have them sign the agreements and place them in the child's file. In the future administration will review agreements at enrollment and ensure all required forms are on file before the first day of attendance.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure that all materials were maintained in a safe and operable condition.

Evidence:
1. In the Pre-School classroom there was a gray area carpet that the binding on the outer edge was torn and pulling away from the carpet creating a trip hazard.
2. In the Infant/Toddler classroom there was a green carpet under the rocking chair. The binding was torn and the uter edge and pulling away from the carpet.

Plan of Correction: Administration removed the rugs and new rugs have been ordered and will arrive by March 28, 2023. In the future as rugs become worn and torn they will immediately be replaced.

Standard #: 8VAC20-780-370-5
Description: Based on observation, the center failed to ensure that awake infants not playing on the floor or the ground a change in play space at least every 30 minutes or more often as determined by the individual infant's needs.

Evidence:
In the Infant/Toddler classroom, Child #3, age 9 months, was observed in an saucer when the Licensing Inspector arrived at 9:20 A.M. The child was not removed or changed positions until approximately 10:00 A.M.

Plan of Correction: Administration will review infant requirements with all staff and will monitor that infants to ensure that staff are changing the position of those children who are not on the floor or ground.
