Comments:
A monitoring inspection was initiated on 3/24/23 and concluded on 3/24/23. There were 40 children present with 9 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 5 staff records were reviewed. The inspection started at 9:30am and concluded at 11:40am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-245-L
Description: Based on review of documentation and discussions with administration the facility failed to ensure that there is always one staff on duty that has current Daily Health Observation Training.

Evidence:
No staff hold current Daily Health Observation Training as required.

Plan of Correction: Staff will be trained in Daily Health Observation as required. At least one staff will be onsite at all times that has this training as required.

Standard #: 8VAC20-780-270-A
Description: Based on review of the playgrounds the facility failed to ensure that areas were maintained in a safe condition.

Evidence:
On the big playground, between the swings and big climbing structure, there was a landscaping timber used to hold the mulch in the paly area that was sticking up, splintered, and causing a potential tripping and impalement hazard to the children.

Plan of Correction: This timber will be fixed or removed to ensure the safety of the children.

Standard #: 8VAC20-780-330-B
Description: Based on review of the newly added swings the facility failed to ensure that the required trajectory zone for the swings was met.

Evidence:
The trajectory zone in front and behind the swings did not measure double the height of the swing cross bar as required. Also, there was not a 6 foot fall zone on the sides of the swings as required.

Plan of Correction: The swings will either be relocated to ensure all the required zones are met, or it will be removed from the playground if the zones cannot be met with relocation.

Standard #: 8VAC20-780-560-M
Description: Based on observation of the children on the playground the facility failed to ensure that children do not eat or drink while walking around.

Evidence:
On the playground a child was seen walking around with a thermos and drinking from it.

Plan of Correction: Staff will ensure that no child is mobile while eating or drinking either inside or outside.
