Comments:
A monitoring inspection was conducted on March 27, 2023 between the hours of 8:45am and 11:45am. There were 28 children present, with 7 staff supervising within the supervision guidelines. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, child and staff records, medication, special care and emergencies and nutrition. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-130-E
Description: Based on record review, the center failed to obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence: Child #4 (DOB 10/25/21) record did not have additional immunizations. Last update was 4/26/22.

Plan of Correction: I will notify the parent that we need the updated immunization record.

Standard #: 8VAC20-780-140-A
Description: Based on record review, each child did not have a physical examination by or under the direction of a physician within 30 days after the first day of attendance.
Evidence: Child #1 (date of enrollment 8/1/22) did not have documentation of a physical examination by or under the direction of a physician at the time of inspection (3/27/23).

Plan of Correction: I will speak to the parents and tell them I need the physical examination immediately.

Standard #: 8VAC20-780-60-A
Description: Based on record review, the center failed to maintain and keep a separate record for each child enrolled which shall contain documentation of viewing proof of the child's identity and age.
Evidence: Child #2 did not have documentation of viewing proof of ID and age in their file.

Plan of Correction: We will notify the parents today and have them bring in the ID.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.
Evidence: The large cabinet in the Little Ducks classroom (ages 2-3 years) was unlocked with a bottle of bleach water solution sitting on the bottom shelf.

Plan of Correction: I will remind the teachers to lock the cabinets. Violation was corrected during the inspection.

Standard #: 8VAC20-780-370-5
Description: Based on observation and interview, the center did not provide awake infants a change in play space at least every 30 minutes or more often as determined by the individual infant's needs.
Evidence:
1) The licensing inspector observed all the children in the Caterpillar / Infant classroom (ages 11-15 months) were confined for 35 minutes. Seven children were in high chairs and 3 children were in bouncers from 8:45am - 9:20am.
2) Seven children were confined in high chairs as they were waiting for breakfast. Breakfast arrived at 9:20am.
3. Staff #7 stated the children were sitting in the high chairs because they were waiting on breakfast and it was late.

Plan of Correction: I will speak to my staff that children are not to be put in high chairs until breakfast arrives. We will ensure they are moved every 30 minutes.

Standard #: 8VAC20-780-500-A
Description: Based on observation, children's hands were not washed with soap and running water after toileting and any contact with blood, feces, or urine.
Evidence:
1. The inspector observed four children's hands were not washed after diapering in the Caterpillar's classroom.
2. Staff #5 did not wash Child #6 and Child #3 hands after diapering.
3. Staff #6 did not wash Child #8 hands after diapering.
4. Staff #7 did not wash Child #7 hands after diapering.

Plan of Correction: I will retrain the teachers again and ensure they are properly washing children's hands. Staff corrected the violation during the inspection.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the staff did not clean the diapering surface with soap and water after each use.
Evidence:
1. Staff #5 did not clean the diapering surface with soap and water after diapering Child #3 and Child #6. Staff #5 only used a bleach solution to clean the diapering surface.
2. Staff #6 did not clean the diapering surface with soap and water after diapering Child #8. Staff #6 only used a bleach solution to clean the diapering surface.
3. Staff #7 did not clean the diapering surface with soap and water after diapering Child #7. Staff #7 only used a bleach solution to clean the diapering surface.

Plan of Correction: I will retrain my staff on the diapering procedures for cleaning and sanitizing the diapering surface. Violation was corrected during the inspection.
