An unannounced monitoring inspection was conducted on 03/10/2023 and concluded on 03/29/2023 in response to a self-reported incident regarding infant feeding. Based on record review, staff interviews, and a review of the centers policies, violations are noted on the Violation Notice.

Standard #: 8VAC20-780-40-E Description: Based on a review of written center procedure and staff interviews, the center did not ensure that the operational responsibilities of the licensee shall include, but not be limited to, ensuring that the center's activities and services are maintained in compliance with the center's own policies and procedures that are required by these standards.
Evidence:
1. Infant feeding procedures : Center procedures for bottle feeding states "to verify the name on bottle with child needing to be fed."
2. On February 28, 2023, Staff #1 fed Child #2 's bottle (formula)to Child #1.
3. Staff #1 and Staff #2 confirmed the wrong bottle was fed to Child #1.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-340-A Description: Based on interviews conducted, staff supervising children did not always ensure their care and protection.
Evidence:
1. Staff #1 confirmed that she fed child #1 2 ounce bottle of formula . The bottle was for child #2 when checking the label.
2. Staff #1 confirmed child #1 is fed breast milk not formula.
3. Staff #1 stated the bottle was out for child #2 . But child #1 was crying she picked up child #1 and then picked up a bottle and fed child #1.Plan of Correction: Not available online. Contact Inspector for more information.
