A monitoring inspection was initiated on March 23, 2023 and concluded on March 30, 2023. There were 11 children present, ranging in ages from 3 to age 5, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed and the background check of 4 other persons.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-4 Description: Based on record review and staff interviews, the licensee did not ensure, all staff had obtained an out of state criminal history name check (CRC) prior to the first day of employment and an out of state child abuse and neglect search (CPS) was request by the 30th day of employment
Evidence: The following out of state background checks were not conducted as required.
1) Staff #1?s record did not include an out of state CPS check and a CRC for Nevada and California where they had lived within the last five years from date of hire (DOH). Staff #1 DOH was 11-28-22.
2) Staff #3?s record did not include an out of state CPS check and a CRC for California where they had lived within the last five years from date of hire (DOH). Staff #3 DOH was 12-12-22.Plan of Correction: Licensee will obtain out-of-state background check CRC before first day and CPS before 30th day of employment. If unable to obtain, a record of phone calls and emails will be kept.
Standard #: 8VAC20-780-240-E Description: Based on record review and staff interviews, the licensee did not ensure each staff member had obtained within 30 days of the first day of employment, training in first aid and cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care.
Evidence: Staff #3 (hire date 12-12-2022) did not have documentation of CPR / First Aid orientation.Plan of Correction: Licensee will make sure to get employees first-aid & CPR before 30th day of employment.
