Provided consultation on standards:
1) 70: for staff hired after 3/1/96, documentation that two or more references as to character and reputation as well as competency were checked before employment
2) 240: Documentation of orientation training shall be kept by the center in a manner that allows for identification by individual staff member, and shall include: name of staff, training topics, training delivery method, entity or individual providing the training, and the date of the training. Best practices is to have the employee and individual providing the training sign the documentation to verify it?s completion.
3) 540: A first aid kit shall be in each building used by children; the required kits shall include tweezers

Comments:
An unannounced renewal inspection was conducted from 9:45 - 11:45am with the center director. There were 52 children, ranging from three years to five years of age, present with seven staff supervising. Children were observed engaged in teacher-directed literacy activities, playing with age-appropriate toys in a variety of centers and completing seasonal arts and crafts projects. Four child records and four staff records were reviewed, along with the emergency drill log, written daily attendance, written allergy list, two medications and authorization forms, the medication log, first aid kit and emergency supplies, and the fire and health inspections. Violations:
Standard #: 8VAC20-780-160-A
Description: Based on review of four staff records, the facility failed to obtain documentation of a negative tuberculosis (TB) test/screening at the time of employment and prior to coming into contact with children. Evidence: the records for staff #1 (date of hire: 5/23/22) and staff #2 (date of hire: 12/28/22), who were both observed in a classroom working with children, did not have documentation of a negative TB test/screening.

Plan of Correction: Both staff will get a TB test/screening.

Standard #: 8VAC20-780-510-E
Description: Based on review of two medications and authorizations, it was determined that the facility failed to follow their medication procedures as required. Long term prescription drug use and over-the-counter medication may be allowed with written authorization from the child?s physician and parent. Evidence: the record for child #1 did not contain written authorization from the parent for long term medication observed on site.

Plan of Correction: Will obtain an authorization form from the parent.

Standard #: 8VAC20-780-540-E
Description: Based on observation, and interview, the facility failed to have a working battery-operated radio in each building used by children. Evidence: the battery-operated radio was not working at the time of the inspection.

Plan of Correction: Batteries were added and the radio now works.
