A monitoring inspection was conducted on 03/28/2023 from 10:10 am until 11:10 am. There were 32 children present, ranging in ages from two months to four years, with seven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 8VAC20-780-60-A Description: Based on the review of five child records, the program failed to ensure required information was in the record for two children.
Evidence:
1. The record for Child #3, with first day of attendance 02/27/2023, was missing the physician's name and phone number, and two emergency contacts.
