Comments:
An unannounced on-site monitoring inspection was conducted on 3/23/2023. There were 35 children, ages 10 months-5 years, and 11 staff members present during the inspection. The inspector observed the following: free choice activities, large motor play outside, lunch and napping. A total of 4 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: attendance requirements. The inspector arrived at 11 AM and departed at 1:30 PM

Violations:
Standard #: 8VAC20-780-70
Description: Based on review of eight staff?s records, the center failed to ensure that all required item per the standard.

Evidence:
1.Staff # 4 did not have the two required references. Staff # 4 start date was 03/08/2021.

Plan of Correction: The director will look for the missing references or will contact the references again.

Standard #: 8VAC20-780-80-A
Description: Based on review of the attendance and interviews with staff, the center failed to ensure that for each grouping of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence:
1.Upon arrival into the center at 11 AM. The toddler class did not have the daily attendance completed. The daily attendance was missing three of the eight children present.

Plan of Correction: The attendance in the futures will be taken as children arrive and depart.

Standard #: 8VAC20-780-270-A
Description: REPEAT VIOLATIONS

Based on observation, the center failed to ensure that all areas and equipment of the center shall be maintained in a clean, safe and operable condition.

Evidence:
1.Two windowpanes in the toddler room had cracks in the glass. This posed a hazard to the children in care.

Plan of Correction: The windowpanes will be replaced and all areas will be monitored.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure that all chemicals were locked in a way that prevents entry.

Evidence:
1.In the upstairs preschool classroom and the toddler room, the locking method did prevent entry into the closet where all the chemicals are stored. The LI could pull the door open when the lock was in place.

Plan of Correction: The closet doors will have a new lock that will ensure chemicals are not accessible.

Standard #: 8VAC20-780-280-G
Description: Based on observation, the center failed to ensure that all chemicals that were stored in a substitute container was labeled.

Evidence:
There was a clear glass container in the toddler closet that was not labeled and did not indicate their contents.
Staff # 4 confirmed that the container contained bleach water.

Plan of Correction: The container will be labeled. All chemicals will be monitored.

Standard #: 8VAC20-780-500-B
Description: Based on observation, center failed to ensure that all diapering procedure were followed according to the standard.

Evidence:
1.The changing pad in the infant room that is being used had tears in the pad. The diaper changing pad is to be non-absorbent surface.
2.The trash cans that are used for disposing of soiled diapers did not ensure that the soiled diaper nor the staff?s hands touched the exterior surface of the storage system during disposal.

Plan of Correction: A new diaper changing pad and new trash cans will be purchased.
