Comments:
An unannounced monitoring inspection was conducted today from 11:40am-1:30pm. There were 136 children (2yrs.-5yrs.) directly supervised by 27 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, circle time, playing with blocks, reading books, playing outside, and eating lunch. There was an adequate supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice.

Description: Based on review of medication and interview, medication with an expired authorization form was not returned to the parent within 14 days of expiration.

Evidence:
The medication authorization form for a medication for child#6 expired on 10/3/2022. The medication was observed on-site during the inspection.

Plan of Correction: we will check all medication and medication forms on a monthly basis for the expiration date. We will return all medications that have expired authorization forms to the families immediately,
