The licensing inspector initiated an unannounced inspection on 03/28/2023 and concluded on 03/28/2023 in response to a self reported incident on 03/17/2023 regarding supervision and physical plant. The investigation was completed on 4/3/2023. Interviews and observations were conducted relating to the allegations. The area of noncompliance with applicable standards or law and violation was documented on the violation notice issued to the facility. Standard #: 8VAC20-780-340-F Description: Based on interview and observation of the playground, the center failed to ensure children under 10 years of age always are within sight and sound supervision of staff.
Evidence:
1. Three children, age three, left through a gap in the fence in the playground. The staff saw the children leave and immediately went to get the children. The fence is tall and staff could not see the children through the fence.
2. Staff 1 stated she saw the children squeeze through the fence and immediately asked for help from coworkers to go get the children. They did not look over the fence and watch the children until someone was able to get to the them.Plan of Correction: All staff implementing classroom supervision at any given time of operation are responsible.
1. As of April 7th, the gap in the fence has been corrected by the fence company.
2. Administration is now conducting routine (roughly by the hour) classroom and playground check ins for ratio confirmation.
3. Station rotations for staff have been implemented outside; staff have designated sections to be present at on the playground and will rotate every 10 minutes to maintain fresh eyes and prevent staff clusters.
4. Playground supervision training and clear expectations of these implementations were reiterated on April 6th at our Staff Meeting.
