A monitoring inspection was initiated on 4/4/2023 and concluded on 4/4/2023. There were seven children present with four staff supervising, one kitchen staff and one observer from the main office. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records were reviewed. The inspection started at 10:00 am and concluded at 11:15 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Standard #: 8VAC20-780-140-A Description: Based on record review the center failed to ensure that Each child shall have a physical examination by or under the direction of a physician: before the child's attendance or within 30 days after the first day of attendance.
Child record #1 has a start date of 2/1/23, no physical examination is on file.
Plan of Correction: The center teacher will contact the parent and get this physical.
Standard #: 8VAC20-780-260-A Description: Based on record review the center failed provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction.
The most current fire inspection is dated 2/2/2022.
Plan of Correction: Contact was made with the fire inspector requesting an inspection be conducted.
Standard #: 8VAC20-780-280-B Description: Based on observation the center failed to ensure that Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
Located in the purple bathroom the cabinet above the sink has cleaning materials. This cabinet was not locked.
