Comments:
A monitoring inspection was initiated on April 5, 2023 and concluded on April 6, 2023. There were 54 children present, ranging in ages from 5 months to age 5, with 15 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-240-C
Description: Based on record a review and staff interviews, the licensee did not ensure each staff member had completed orientation training in all required facility specific topics prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
Evidence: The record for Staff #1 (hire date 3-13-23) did not include information indicating orientation training had been conducted related to prevention of shaken baby syndrome, abusive head trauma and procedures to cope with distraught children including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness was conducted.

Plan of Correction: A new form has been created to reflect that these topics are included in orientation. Previous form needed to be updated as the info is included during onboarding orientation. The new form will be used moving forward.

Standard #: 8VAC20-780-510-G
Description: Based on observation and staff interviews, the licensee did not ensure that medication was labeled with the name of the medication, the dosage amount, and time or times to ne given of medication.
Evidence: An epinephrine auto injector was observed designated for child #1 and it was not labeled with the child?s name, medication dosage amount, times or time to be given.

Plan of Correction: Spoke with parent of child w/ EpiPen letting them now we also need the packaging w/ prescription label on 4-5-2023. Parent brought box w/ label & instructions on 4-6-2023.

Standard #: 8VAC20-780-510-P
Description: Based on review of documentation and staff interviews, the licensee did not ensure when an authorization for medication expires, the parent shall be notified the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.
Evidence: Child #5 had a medication and the medication authorization signed by the parent expired on 1-21-23. The medication had not been sent home to the parents or disposed of as required

Plan of Correction: Medication returned to family. Reviewed w/ staff to send medications home when form is expired, or have family complete a new form to keep current and med can remain at center.
