An unannounced monitoring inspection was initiated on March 24, 2023 at 9:15 a.m. and concluded on April 6, 2023. Two staff interviews were conducted and documentation from one staff record was reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-200-A Description: Based on review of documentation and interview, the center failed to have a qualified program director.
Evidence:
1. Staff #1 and Staff #2 confirmed that Staff #1 is the current program director. The documented education and programmatic experience in Staff #1?s record did not indicate that Staff #1 was qualified as program director.
2. Staff #1 and Staff #2 were instructed to send any additional documentation to the Department by noon on 04/04/2023. Documentation received did not demonstrate that Staff #1 qualified as a program director.
