Discussed the Allergy list and care plans. Discussed the capacity and modification process.

An unannounced monitoring inspection was conducted on 4/06/2023 from 10:15am to 12:35pm. At the time of entrance, 35 children were in care with 8 staff members present. Children were observed playing outside, eating snack, reading books, washing hands, coloring and dying easter eggs and playing with puzzles. Walls, bulletin boards and several other areas of the programming space was decorated with artwork, crafts and projects completed by the children. Interactions between the children and staff were positive. The site was organized and contained appropriate materials. A selection of staff and children records, medications, the physical space, evacuation drills, emergency supplies and attendance records were reviewed. Description: Based on review of staff records, the center did not obtain the central registry findings within 30 days of employment for one staff member.
1. Staff #1 (Date of hire 8/22/2022) did not have the central registry findings in the center's files.

Plan of Correction: Central Registry findings will be obtained.

Description: Based on review of two staff records, the center did not obtain documentation of a negative tuberculosis screening for each staff member prior to hiring and within the last 30 calendar days of the date of employment.
1. Staff #1 (Date of hire 8/22/2022) had a TB test or screening dated 9/16/2022.
2. Staff #2 (Date of hire 1/17/2023) had a TB test or screening dated 1/21/2023.

Plan of Correction: Will obtain TB test results prior to hiring.

Standard #: 8VAC20-780-70
Description: Based on record review, required records were not kept for each staff person.
1. Staff #2 (Date of hire 1/17/2023) was listed as a lead teacher, but the record did not have documentation to demonstrate that the individual possesses the education or certification required by the job position.

Plan of Correction: Will obtain a copy of diploma/transcripts.

Standard #: 8VAC20-780-240-A
Description: Based on review of staff records, one staff member did not complete the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.
1. Staff #1 (Date of hire 8/22/2022) had not completed the Virginia Department of Education-sponsored orientation course.

Plan of Correction: Staff shall complete preservice training.

Standard #: 8VAC20-780-260-A
Description: Based on review, the center did not provide to the licensing representative an annual fire inspection from the appropriate fire official having jurisdiction.
1. The last fire inspection was dated 9/23/2021.

Plan of Correction: Will re-schedule fire inspection.
