Technical Assistance:
Discussed with the provider: TB tests must be submitted by the employee?s first day of employment and must not be more than 30 days old. Inspection summary and violation notice must be posted in a way that allows all pages to be viewed. The supplemental page is considered confidential and should not be posted. All medications must be labeled with the child?s name, even if they are in a labeled bag. The next anticipated inspection will be the providers renewal inspection. Provider was reminded to have all renewal application documents completed and submitted to the licensing office 45 days prior to expiration for processing. The renewal application is available on the VDOE website. The new application must be used.

Comments:
An unannounced Monitoring Inspection took place on 04/11/2023 between the hours of approximately 9:00 a.m. and 11:10 a.m. There were 6 classrooms observed with a total of 78 children with 15 staff within the supervision guidelines. The children were observed participating in circle time, playing in organized centers, napping, and preparing to go to the playground. Positive interaction between staff and children were observed. A complete inspection of the physical plant, children and staff records, fire drill log, medication and injury prevention plan was conducted during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-770-60-B
Description: Based on record review, an employee of the center was employed before the center had the person?s sworn statement.

Evidence:
1. Staff #1 (start date: 09/26/2022) had a Sworn Statement dated 09/27/2022.
2. Staff #4 (start date: 03/13/2023) had a Sworn Statement dated 03/29/2023.

Plan of Correction: Going forward we will have it by the date of hire.

Standard #: 8VAC20-780-130-A
Description: Based on record review, the center did not obtain documentation that each child received required immunization before they could attend the center.

Evidence: At the time of inspection, Child #1 (start date: 07/18/2022) did not have documentation of immunizations on file.

Plan of Correction: We will call the parents this afternoon and follow-up at pick-up. They must give turn it in by Friday or they cannot come in.

Description: Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence: Child #4?s (age: 21 months) most recent documentation of immunizations was dated 09/15/2022.

Plan of Correction: We will call the parents this afternoon and follow-up at pick-up. They must give turn it in by Friday or they cannot come in.

Description: Based on record review, not each staff member submitted documentation of a negative tuberculosis screening at the time of employment.

Evidence:
1. Staff #1 (start date: 09/26/2022) had a tuberculosis screening that was dated 11/09/2022.
2. Staff #4 (start date: 03/13/2023) had a tuberculosis screening that was dated 03/21/2023.

Plan of Correction: Going forward we will have it by the date of hire.

Description: Based on record review, the center did not obtain all information for children?s records.

Evidence: At the time of inspection, Child #1?s (start date: 07/18/2022) record did not contain documentation of viewing proof of the child?s identity and age.

Plan of Correction: We will call mom and have her bring it by today.

Description: Based on record review, not all staff, hired after March 1, 1996, had documentation that two or more references were checked before employment.

Evidence:
1. At the time of inspection, Staff #1 (start date: 09/26/2022) did not have documentation that two or more references were checked.
2. At the time of inspection, Staff #2 (start date: 02/13/2023) only had documentation that one reference was checked.
3. At the time of inspection, Staff #4 (start date: 03/13/2023) did not have documentation that two or more references were checked.

Description: Based on observation, cosmetics or other harmful agents were stored in areas accessible to children.

Evidence:
1. There was a bottle of hair mousse (warning label says to keep out of reach of children) in a yellow bin on a shelf, inside the Preschool classroom?s bathroom, accessible to children.
2. In the Pre-K 1 classroom, there were five cans of shaving cream (warning label says to keep out of reach of children), in a bin on a wire rack by the sink, accessible to children.

Description: Based on observation, there was not at least 12 inches between the sides of occupied cribs.

Evidence:
1. At the time of inspection, Child #6 and Child #7 were napping in cribs that were located next to each other.
2. There was approximately seven inches between the sides of Child #6 and Child #7?s cribs.

Description: Based on observation, sunscreen was accessible to children under five years of age.
