A focused monitoring inspection was completed on Tuesday, April 11, 2023.There were 46 children present with 9 staff supervising including an administrative staff on site. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22.1-289.035-B-4 Description: Based on review of staff records and interview, the center did not ensure to obtain a copy of the results of a search of child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.,
Evidence: 1. In Staff#1's record (date of hire: 12/22/2022), there was no results of a central registry finding found. 2. In Staff#3's record(date of hire: 01/27/23) there was no results of a central registry finding found. 3. Interview with staff confirmed results from out of state central registry searches were not available for review during the inspection.Plan of Correction: Not available online. Evidence:
1. In Child#1's record one of the emergency contact address was incomplete.
2. In Child#3's record one of the emergency contact address as incomplete.
3. In Child #4's record both of the emergency contacts addresses were incomplete.
