The licensing inspectors conducted an unannounced inspection in response to a self-reported incident involving a serious injury of a child. On April 6, 2023, the inspection was initiated by the licensing inspector. During the inspections, interviews were conducted with the staff, and observations of physical plant and injury report.
No violations were cited.
