Comments:
An unannounced renewal inspection was conducted on April 11, 2023, to determine the center?s compliance with licensing standards. There were 26 children present, ranging in ages from 4 months to 5 years, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care, transportation, emergencies, and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 8VAC20-780-60-A
Description: Based on review, and interview with the director, two of five children's records did not contain complete emergency contact information. Evidence:
1. The record for child # 1, did not include an address or telephone number 1 of 2 emergency contacts.
2. The record for child # 5, did not contain a name, address, or contact number for 1 of 2 emergency contacts.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not ensure all areas and equipment were maintained in a clean, safe, and operable condition.
Evidence:
1. The first step on Bus #1, is lose. The front of the step as a strip that is approximately 4-5 inches long, that has separated from the step.
2. The director was aware that the step was lose and stated that the step will be repaired.

Description: Based on observation and interviews, it was determined that there was an insufficient amount of resilient surfacing in the use zone of the playground equipment and the area
surrounding equipment.
Evidence:
1. Under the yellow slide on the preschool playground, there is approximately 1-2 inches of mulch.

Description: Based on record review of a sampling of written record of children's serious and minor injuries, injury reports did not contain all required documentation.
