Comments:
A monitoring inspection was conducted on April 14, 2023 from approximately 10:10 am to 1:20 pm. A total of five child records and 4 staff records were reviewed. The center's director was available throughout the inspection to answer questions.

There were 5 children present, ranging in ages from 2 to 3 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition.

During the inspection, the children and staff were observed participating in activities and the staff were observed having positive interactions with the children.

Areas of the facility including all classrooms and bathrooms were inspected. No medication is administered and no meals are prepared. The center is equipped with toys and supplies and items were available to the children.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.


Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Just writing the word corrected is not
acceptable.

Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Standard #: 8VAC20-780-160-A
Description: Based on staff record review and interview, the center did not ensure that an employee submitted documentation of a negative tuberculosis screening that had been completed within the last 30 calendar days of the date of employment.

Evidence:
The tuberculosis screening report in the record for Staff #4 (date of employment: 11/21/2022) was dated 09/21/2022 which is beyond 30 calendar days of the date of employment.

During an interview with the site director, the date of employment and date of the tuberculosis screening for Staff #4 were acknowledged as being correct.

Description: Based on observation, the center failed to ensure the findings of the most recent
inspection were posted.

Evidence:
The most recent inspection findings posted at the center was an acknowledgment of
inspection form dated 08/04/2022.

During an interview, the director,acknowledged the most recent inspection findings were not
posted.
