22.1 Early Childhood Care and Education

Technical Assistance:
When children are in ongoing mixed age groups, the staff-to-children ratio and group size applicable to the youngest child in the group shall apply to the entire group.

Comments:
An unannounced renewal inspection was conducted beginning at 4:15 p.m. and ending at 5:30 p.m. There were 29 children in care ages 5 years through 11 years old under the care of two staff. Programming including snack time, transition, handwashing, bathroom break were observed.

Five children's records and two staff records were reviewed. Five medications and authorizations were reviewed. Emergency drills were posted and up to date.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Based on review of documentation, not all records for children with a diagnosed food allergy had a written care plan from a physician. Evidence: Child #9 has a diagnosed food allergy to nuts and there was no written care plan for the diagnosed food allergy, that includes instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction

Description: Based on observation and staff interview, the required ratio for children ages 5 to 9 years old of 1 staff for every 18 children was not always followed. Evidence: On March 30, 2023, there were 25 children ages 5 years through 11 years of age under the supervision of two staff. During hand washing and bathroom break, staff was observed leaving one staff alone with 25 children to answer the door for a parent pick up in the main childcare area in the gym that was out of sight and sound supervision of the hallway area where the children and other staff were lined up for approximately three minutes. Once hand washing was completed, Staff #2 was observed leaving the group in the main childcare area in the gym to use the rest room, leaving the 24 children with one staff for approximately five minutes. Staff interview confirmed that for the last week the program has operated this way.

Description: Based on review of medication it was determined that the center failed to obtain authorization for prescription medication as required. Evidence: Child #10, had an epi pen on site and the authorization from the parent expired on November 2022 and from the physician on 1/18/2023.
