A (monitoring/renewal/complaint) inspection was conducted on April 17, 2023. There were twenty-eight children present with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and two staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-60-A Description: Based on review, facility did not ensure 2 of 5 children's records have documentation of required information.
Evidence: The following was found during record review:
1. The record for child #3 (start date 12/4/2022) did not have parent employment information and emergency contact addresses.
2. The record for child #5 (start date 3/23/2023) did not have parent's employment phone number and emergency contact addresses.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-280-B Description: Based on observation, facility did not ensure hazardous substances such as cleaning materials are kept in a locked place using a safe locking method that prevents access by children.
Evidence: Cleaning supplies were observed stored on top of a refrigerator in the cafeteria. Hand sanitizer was observed stored on a table in the cafeteria.Plan of Correction: Not available online. Contact Inspector for more information.
