An unannounced renewal inspection was conducted on 04/19/2023 from 10:30am to 1:30pm. There were 82 children ages 4 months to age 5 years old supervised by 17 staff. The physical plant, outdoor playground area, programming, attendance, menus, 5 staff records, 5 children?s records, 17 medications with corresponding authorization records, emergency drills, injury reports, emergency supplies, and policies were reviewed. Children were observed participating in floor play, table toys, art, lunch, building activities, reading and outside play. Diapering and handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Description: Based on review of the written attendance record and interview with staff, the center did not ensure they maintained a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs.

1)During inspection in the Phonics Enrichment classroom, there were 5 children present, but there was no written record for the children present in this classroom. An electronic version of the record showed 4 children present.

2)During inspection in the Infant classroom, there were 11 children present, but the written record reflected there were 10 children present.

Plan of Correction: Retrain on child supervision records/recording attendance.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the center did not ensure that disposable diapers were disposed in a storage system that is either foot-operated or used in such a way that neither the staff member?s hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Based on observation the center did not ensure that the diapering surface was cleaned and sanitized after each use.

1)In the Toddler classroom, the foot-operated storage system for disposal of diapers was not being operated by foot causing the staff member to touch the exterior surface of the trash can with a soiled diaper to open the trash can.

2)In the Toddler classroom, a staff member changed a child's diaper then sprayed a cleaner on the diapering surface and wiped it clean. No sanitizer was used as required. To properly sanitize the diapering surface, the sanitizer should be sprayed on the surface and the solution allowed to dry for at least 2 minutes.

Plan of Correction: Review diapering procedures.
