Technical Assistance:
The provider must send documentation to the LI that the background checks have been requested no later than 10 days following this notification dated April 19, 2023. Once the background check is received by the center, the results must be forwarded to the licensing inspector.


Discussed upcoming expiring criminal record checks.


Discussed updated requirements to staff orientation and emailed a model form of the updated requirements which went into effect 10/13/2021.

Discussed the need for a dosage on the authorization form for a medicated ointment.

Discussed the requirement to conduct a lockdown drill annually in addition to two shelter-in-place drills per year, and monthly evacuation drills.

Comments:
An unannounced monitoring inspection was conducted today from 10:30am-12:30pm. There were 58 children directly supervised by 10 staff. The physical plant, 7 staff records, 7 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, playing outside, napping, and reading. There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Description: Based on record review, one staff did not have the results of a national criminal record check within the preceding five years.
Evidence:
The criminal record check for Staff 2 was conducted on 3/23/2018.

Plan of Correction: Fingerprinting process completed on staff 2.

Standard #: 22.1-289.035-B-4
Description: Based on record review, the center did not have the results of a criminal record check for a state an employee member was a resident of in the preceding five years.
Evidence:
Staff 6's record does not contain the results of a criminal record check for a state she has lived in within the preceding five years.

Description: Based on record review, the center did not develop written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.
Evidence:
On the date of inspection the director could not produce written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.

Plan of Correction: Written policies on Shaken Baby Syndrome have been written. These policies are located in each of the infant rooms and in the director?s office. The staff have signed off on these policies indicating they have read the policies regarding SBS. SBS signs have been posted for parent?s reference. Staff completed Shaken Baby Syndrome training the week of April 24, 2023.

Description: Based on record review, one child's file from the sample reviewed did not contain all required information.
Evidence:
Child 4's record did not contain a written care plan for a diagnosed food allergy, to include instructions from a physician regarding food to which the child is allergic, and steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: There was not any written instructions from the child?s doctor. On the child?s completed physical form the doctor signed off that the allergic reaction, is a rash breakout/local reaction. The doctor deemed on this form under response required-none. Doctor signed off on this on 3/21/23.

Standard #: 8VAC20-780-240-A
Description: Based on record review, two staff hired after 10/13/2021 had not completed the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.
Evidence:
Staff 1 and Staff 2's records do not contain a completion certificate for the Virginia Department of Education-sponsored orientation course. Staff 1 was hired in November 2021. Staff 2 was hired November 2022.

Plan of Correction: Staff 1 and Staff 4 have started the 10-hour orientation course 4/24/23.

Standard #: 8VAC20-780-240-B
Description: Based on record review, staff did not complete orientation training prior to the staff working alone with children and no later than seven days of the date of assuming job responsibilities.
Evidence:
Staff 1's record does not have a completed orientation form on file.

Plan of Correction: Staff have signed off on new forms and are in their files.

Standard #: 8VAC20-780-270-A
Description: Based on observation, areas of the center were not maintained in a safe condition.
Evidence:
1. In the two year old classroom, a wooden toy microwave cabinet was observed to have cracked and splintering wood on the back corner in an areas accessible to children.
2. The wooden planks at the base of the playground ramp were observed to be detaching from the ramp and exposing the finishing nails used to hold the planks in place.
3. Two steak knives were observed in an unlocked cabinet in the Young toddler classroom.

Plan of Correction: Our facility is under the care of the federal government. The ramp has been observed by those that preform maintenance on our facility. They have submitted a work order weeks ago regarding this, however approval on this type of construction takes time within the federal government. Completion date unknown

Repeat: Teachers have been instructed on safety practices and are required to take a refresher course on classroom safe environments. The teachers are required to take this training and complete it no later than 4/28/23.

Standard #: 8VAC20-780-570-E
Description: Based on observation, infant bottles were not labeled with the child's name and the date.
Evidence:
A bottle was observed in the refrigerator in the Infant classroom without a label indicating the child's name or the date.

Plan of Correction: Staff were instructed to please check bottles before parents leave the facility.
A reminder notice was posted to parents outside of each infant classroom listing the parent?s responsibility on labeling the bottles and any food brought in from home- 4/21/23.
