Discussion on staff training on standards and supervision of children during transitions.

Comments:
A self reported complaint was received in the licensing office on February 9, 2023, regarding a 3-year-old was left behind on the indoor playground. On April 21, 2023, an on-site investigation was conducted. Interviews were conducted and observations were made.

Based on the preponderance of evidence gathered, violations were cited.

Standard #: 8VAC20-780-40-E
Complaint related: Yes
Description: Based on review of records and video camera footage, the center did not ensure that the center?s activities and services and facilities were maintained in compliance with required by standards.
Evidence:
1.On February 8, 2023, Staff #1 did not follow center?s playground safety policy to use the name to face recognition as each child crosses the threshold into the classroom.
2. Staff acknowledged she did not use Tadpole for name to face recognition as each child entered the building from the infant playground.
3. The center?s playground safety policy requires that staff use Tadpole to complete the name to face recognition process when children are in transition from the playground.

Plan of Correction: Training with staff to always ensure sight and sound of children.
We will ensure face to name process are appropriate at each threshold and mainly appropriate when moving children form one space to another.
Administration will do spot checks as staff perform the name to face recognition and move from space to the next.
Retrain staff who are not appropriately preforming name to face recognition process.
Make sure final name to face recognition is done at group?s final destination.
Upcoming staff meeting May 2023 and will review with staff during this time as well.

Description: Based on interviews, documentation and video recordings, the center did not ensure that when staff were supervising children, they always ensure their care, protection, and guidance.
Evidence:
1.On February 8, 2023, Staff#1 did not follow the center?s playground safety policy to use name to face recognition using Tadpole during transition and at every threshold before returning to the classroom.
2.Staff #1 went inside the three-year-old classroom with 9 children leaving 3-year-old, Child A behind on the infant indoor playground.
3. Child A walked out the infant indoor playground when a parent unlocked the gate and walked behind the parent to the Pre-K classroom.
4. Staff #2 received the child from the parent because the classroom of three-year-olds and Pre-K had combined classes.
5. Staff #1 did not notice Child A was not with the group and did not notify the center?s administration of the incident before leaving for the day.

Description: Based on interviews and video camera recordings, the center did not ensure that children less than 10 years of age were always in sight and sound supervision of staff.
Evidence:
1. On February 8, 2023, a group of 10 three-year-old children were on the Pre-K indoor playground area from 4:30 pm to 5:00 pm with Staff #1.
2. At approximately 4:55 pm, Staff #1 had the children line up to return to the classroom. The group would have to walk through the pre-k playground gate to the infant playground and then through another gate to the hallway leading to the classroom. Staff #1 had the Tadpole system (used for name to face recognition) in hand before opening the gate to transition back to the classroom.
When Staff #1 opened the indoor infant playground gate the children ran down the hallway to the classroom.
Staff #1 called for the children ahead while walking to the classroom, and then went inside the classroom with 9 children.
3.Child A (three-year-old) had stopped walking and stood against the wall on the infant playground as Staff #1 continued to the classroom. Child A was left alone and unsupervised on the infant indoor playground for approximately 1 minute and 8 seconds.
4. A parent was picking up their child from the Toddler classroom. The parent walked through the indoor locked playground gates playgrounds to the Pre-K classroom to pick up another child, and Child A walked behind the parent. Staff #2 who was in the Pre-K classroom received Child A from the parent.
5. At that time, the classroom of three-year-old children and Pre-K had combined classrooms.
