Discussed giving detail in an Injury Report. The Circumstance needs to be explained whether it states type of injury and/or specifically details how it occurred. Be sure to include the type of equipment involved.

A monitoring inspection was conducted on April 20, 2023 after the center self reported two separate injuries that occurred on the playground in the span of one week. Records were reviewed, supervision was observed, and interviews were conducted. Staff were observed redirecting children during outside play. The information gathered during the inspection did not find violations occurred in the two incidents.
