1. The ongoing playground maintenance should include cutting and removing the ground cover cloth under the wood mulch to prevent trip hazards.
2. The s-hooks on the swing chains should be closed to less than the depth/width of a penny. y
3. Recommend all emergency drills be recorded annually on one document, to include the evacuation, lock-down, and shelter-in-place drills. A total of 15 per year are to be recorded.
4. The inspector will provide model forms, as discussed during the inspection.

Conducted an unannounced monitoring inspection at 11:15am. Observed 20 children + 5 direct-care staff + 1 parent volunteer in the morning program. Ratios and supervision were in compliance. Children were observed painting, participating in a music activity, outdoor active play, eating snack, and being dismissed for the day. While children were observed inside the building at times this is predominantly an outdoor education program. Staff were actively engaged with the children and activities. Evidence: Staff #9 left volunteer service with the school in 2020, was hired as staff in 2022, and the central registry search on-file was dated as completed in September 2019. Evidence: A spray disinfectant was sitting on the frame of the wall (a horizontal piece of wood framing the wall) by the toilet in the upstairs bathroom used by children.
