The licensing inspector conducted an unannounced monitoring and complaint inspection on 04/21/2023. The complaint that was received in the licensing was regarding allegations in the area of physical plant. Interviews were conducted with staff, records reviewed and observations of the program were made relating to the allegation. The information gathered during the inspection determined no violations with applicable standards or law, related to the complaint, however other violations were cited.

Standard #: 8VAC20-780-260-B Complaint related: No Description: Based on a record review the center did not ensure an annual health inspection was
conducted of the facility.
Evidence
The last documented health inspection was dated 01/24/2022.Plan of Correction: The Health Department has not been out yet to conduct the inspection. The
director will the department and request an inspection and will document the conversation with the
date.
Standard #: 8VAC20-780-280-B Complaint related: No Description: Based on observation the center did not ensure all hazardous substances were kept in
a locked place using a safe locking method that prevents access by children.
Evidence:
1. The 4 year-old classroom had 2 opened air fresheners, one of children's cubby and
another on top of the supply cabinet.
2. The supply cabinet in the 4 year-old room was unlocked. It contained at least 5 bottles of
disinfectant and cleaning sprays.
3. On the playground area there was a bottle of Miracle Gro Plant foodPlan of Correction: The director will remind staff that all chemicals are to be kept separate from food.
Standard #: 8VAC20-780-280-D Complaint related: No Description: Based on observation the center did not ensure cleaning and sanitizing materials were
not located above food, food equipment, utensils or single-service articles and shall be stored in areas physically separate from food.

In the 4 year-old room several bottles of disinfectant spray were stored above a bag
and large jar of candy.Plan of Correction: The director will remind staff that all chemicals are to be kept in a locked
place.
Standard #: 8VAC20-780-280-G Complaint related: No Description: Based on observation the center did not ensure if hazardous substances are not kept in
the original container, the substitute container should clearly indicate the contents.
Evidence:
There were 3 tinted spray bottles on the playground, both containing a liquid substance
and neither labeled to identify its contents.Plan of Correction: All staff will be retrained on the proper storage and labeling of chemicals.
Standard #: 8VAC20-780-330-B Complaint related: No Description: Based on observation the center did not ensure where playground equipment is
provided, resilient surfacing shall comply with minimum safety standards when tested in
accordance with the procedures described in the American Society for Testing and Materials
standard F1292-99.
Evidence:
The composite structure on the toddler playground did not have the required amount
of resilient surfacing in the fall zone of the structure. There was about 3 inches, where 6
inches is required. In addition, grass was growing in the fall zones reducing the resiliency of the surfacing.Plan of Correction: Resilient surfacing has been ordered. The director is waiting for a delivery
confirmation.
Standard #: 8VAC20-780-560-F Complaint related: No Description: Based on observation the center did not ensure all requirements were met when choosing to provide meals and snacks.
Evidence:
The center did not have a menu posted in the building.Plan of Correction: The director will speak with the cook to ensure there is a menu posted at all
times for at least 1 week in advance.
