Comments:
A monitoring inspection was initiated on April 18, 2023 and concluded on April 19, 2023.
There were 24 school age children present with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-160-C
Description: Based on review of staff record and interview, the center did not ensure staff obtain and submit the results of a follow up tuberculosis screening every two years.

Evidence: In Staff#1's record the last dated TB screening was 12/21/2020. Interview with staff confirmed the TB documentation available for review during the inspection.
