Discussion on staff records and areas and equipment.

Comments:
A monitoring inspection was initiated and concluded on 4/05/2023. There were a total of 42 children children with 13 staff members present. Children were observed resting and eating (taco, beans, pineapple and milk). Staff were actively engaged and interactive with the children.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-210-A
Description: Based on staff records review, the center did not ensure a program leaders meet at least 18 years of age, have fulfilled a high school program completion or the equivalent.
Evidence:
1.Staff #2 (date of employment 9/23/2022) did not have documentation in record of a high school program completion or the equivalent.

Plan of Correction: Staff member provided copy of associates degree.

Standard #: 8VAC20-780-270-A
Description: Based on observation of the classrooms, areas and equipment of the center inside were not maintained in a clean, safe and operable condition.
Evidence:
1.In the classroom of three-year-old children, a red and blue couch had rips in on the arm rest and seat.
2. The toilets in the classroom for 2?s, 3?s and Pre-K had protruding screws on the base of the toilet. The bathrooms are accessible to children.

Plan of Correction: 1. Couch was thrown away and new one ordered.
2. All screws were covered in all bathrooms.
