A monitoring inspection was conducted on 04/25/2023. There were a total of 63 children with 6 staff members present. Children were observed washing hands, engaged in small group centers and large group story times, and dismissal procedures. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22.1-289.035-A Description: Based on a review of staff records, the CDC did not ensure all employees undergo a repeat background check every five years.
Evidence:
Staff #2 had a fingerprint letter dated 4/11/2018.
Staff #2 had a sworn disclosure statement dated 9/26/2017.Plan of Correction: We will have the employee fill out a new sworn disclosure and we will schedule them to get their fingerprints done.
Standard #: 8VAC20-780-245-J-1 Description: Based on a review of staff records and staff interviews, the CDC did not ensure that whenever the center has agreed to administer prescribed medications, that the administration is performed by a staff member or independent contractor who has satisfactorily completed training on administering medications.
Evidence:
There was no one on site that had MAT training in their records.
The Director confirmed that no one on site was MAT trained.Plan of Correction: We are scheduling a MAT training class as soon as possible.
