A monitoring inspection was conducted on 04/19/2023. There were 125 children present, ranging in ages from 8 weeks to 5 years old, with 23 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of ten child records and ten staff records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 8VAC20-780-160-A Description: Based on the review of ten staff records, the program failed to ensure that each staff member submits documentation of a negative tuberculosis screening within the last 30 days of the date of employment for five staff.
Evidence:
1. The date of employment for Staff #2 is 12/5/2022. The TB screening in the record was dated 11/18/2021.
