A, monitoring inspection was completed on Wednesday, April 27, 2023 at the center.
There were 25 children present with 6 staff supervising including administrator. The
inspector reviewed compliance in the areas of administration, physical plant, staffing and
supervision, programming, special care and emergencies and nutrition. A total of
4 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22.1-289.035-A Description: Based on review of staff records and interview, the center did not ensure to obtain all staff background checks every five years thereafter.
Evidence: In Staff#3's record, the central registry findings was dated 06/07/17. Interview with staff confirmed findings available for review during the inspection.Plan of Correction: The center's director will submit request to central registry finding for staff and include copy in file once completed.
