Comments:
An unannounced monitoring inspection was initiated and concluded on 4/27/2023. The inspector was on site from approximately 8:36 am-11:53 am. There were 53 children present, ranging in ages from 2 months to 5 years, with 11 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, nutrition and background checks. A total of 6 child records and 6 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on a review of six (6) staff records and interview, the center did not ensure to obtain documentation of a negative tuberculosis (TB) screening for one (1) staff at the time of employment and prior to coming into contact with children. The documentation must have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence:
1. The record of staff #4 (date of employment: 4/10/2023) contained a TB screening dated 11/27/21.
2. Administration acknowledged that the TB screening did not occur within the required time frame.

Plan of Correction: staff member has made appointment for her TB testing

Standard #: 8VAC20-780-70
Description: Based on a review of six (6) staff records and interview, the center did not ensure that one (1) staff record was kept that contained the required information.

Evidence:
1. The record of staff #4 (date of employment: 4/10/2023) contained references dated 4/12/2023. Staff records are required to contain documentation that two or more references as to character and reputation as well as competency were checked before employment.
2. Administration acknowledged that the references were obtained after employment.

Plan of Correction: moving forward all references will be completed prior to start dates

Standard #: 8VAC20-780-420-E-3
Description: Based on a review of six (6) child records and interview, the center did not ensure that two (2) records contained documentation of parent confirmation that the required information in the child's record was up to date at least annually as required.

Evidence:
1. The records of child #5 (date of attendance: 1/29/21) and child #6 (date of attendance: 1/29/21) did not contain documentation of an annual review/update.
2. Administration acknowledged that there was not documentation of the review.

Plan of Correction: file updates with parents are being scheduled for 5/1-5/12
