A Subsidy Health and Safety inspection was conducted on 4/27/2023 to review supplemental health and safety requirements.
The subsidy requirements were reviewed this date; allergy action plans, staff trainings, nutrition and food services, recordkeeping, hazardous substances, reporting injuries, emergency plans and transportation.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 8VAC20-790-600-D-1 Description: Based on review of staff training, five staff do not have current certification in cardiopulmonary resuscitation (CPR).
Evidence:
1. Staff #1, 2, 3, 4 and 5 all had CPR certification that expired 3/2023.Plan of Correction: I will schedule them as soon as possible.
Standard #: 8VAC20-790-600-D-2 Description: Based on review of staff training, three staff do not have current certification in first aid training.
Evidence:
1. Staff #6, 7, and 8 all had First Aid certification that expired 3/2023.Plan of Correction: I will schedule them as soon as possible.
Standard #: 8VAC20-790-600-F Description: Based on review of training records, 9 staff need to complete the health and safety update course.
Evidence:
1. Staff A completed the health and safety update training on 2/28/2022.
2. Staff B completed the health and safety update training on 2/23/2022.
3. Staff C completed the health and safety update training on 2/18/2022.
4. Staff D completed the health and safety update training on 2/11/2022.
5. Staff E completed the health and safety update training on 2/26/2022.
6. Staff F completed the health and safety update training on 2/20/2022.
7. Staff G completed the health and safety update training on 2/15/2022.
8. Staff H completed the health and safety update training on 2/26/2022.
9. Staff I completed the health and safety update training on 4/05/2022.Plan of Correction: I will follow up with staff to complete the required training.
