The model form for medication authorization is good for one year on the Licensing website.

Comments:
An unannounced monitoring visit was conducted today. All classrooms were in ratio with direct care staff. Observed children on the playground area, room activities such as free play and art projects, transition to handwashing and preparation for lunch (bought from home). Reviewed physical space in classes, records for children, mediation, trainings for staff and emergency preparation. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description: Based on record review, a child's allergy care plan has not been updated annually.
Evidence:
Child #1's allergy care plan expired 3/30/2023.

Plan of Correction: On the first Friday of each month, all medications and forms will be checked.

Standard #: 8VAC20-780-510-E
Description: Based on medication and record review, the center's procedures for medication did not prevent use of outdated medication.
Evidence:
Child #1's emergency medications expired 11/2022.

Based on medication and record review, the center's medication authorizations were expired.
Evidence:
Child #1's medication authorization expired 3/16/2022 for 2 medications and 6/17/2022 for one medication.
Child #2's authorization expired 8/31/2022.

Plan of Correction: All parents of children with medications at school have been contacted and given new medication forms. Requested new medications for any expired ones.

Description: Based on record review and administration statement, the center did not have two shelter-in place drills per year.
Evidence:
Only one drill was conducted the previous school year.

Plan of Correction: the preschool conducted its weather drill on 4/28. Lockdowns scheduled for 5/5 and 5/15.

Description: Based on observation, posted emergency numbers were not in a visible place at each telephone.
Evidence:
No emergency numbers were listed on the Monkey's classroom phone area.

Plan of Correction: New emergency number cards were made and placed nest to each classroom phone.

Standard #: 8VAC20-780-560-G
Description: Based on observation, food containers from home were not clearly dated and labeled in a way that identified the owner.
Evidence:
A random sample of lunch boxes were observed. 4 were not labeled with the child's name and none were dated.

Plan of Correction: All children's lunches will be labeled. Teachers will document on a food calendar each day.
