1) 520C - If diaper ointment or cream is used, the following requirements shall be met: a) written parent authorization noting any known adverse reactions shall be obtained; b) these products shall be labeled with the child's full name; c) these products shall be inaccessible to children; d) a record shall be kept that includes the child?s name, date of use, frequency of application and any adverse reactions. Over-the-counter skin products shall not be kept or used beyond the expiration date of the product

Comments:
An unannounced renewal inspection was conducted from 12:40 - 3:30 pm with staff. There were 78 children, ranging from two months to ten years of age, present with eight staff supervising. Children were observed napping, washing hands, and playing outside on the playground. Seven child records and six staff records were reviewed, along with the written daily attendance, written allergy list, two medications and their authorization forms, the emergency drill log, first aid kit and emergency supplies, liability insurance and fire and health inspections.

Standard #: 8VAC20-780-160-A
Description: Based on review of six staff records, the facility did not have documentation of a negative tuberculosis (TB) test/ screening for all staff at the time of employment and prior to coming into contact with children. Evidence: the record for staff #5 (date of hire: 2/7/2022), who was observed working in a classroom, did not have documentation of a TB test/screening that was dated, demonstrating that it was completed within 30 days prior to employment, or signed by a physician.

Description: Based on review of six staff records, the facility did not have documentation of a negative tuberculosis (TB) test/ screening for all staff at least every two years from the date of the initial screening. Evidence: the record for staff #3 had documentation of a TB test/screening dated: 4/25/2019.

Description: Based on review of documentation, and an interview with staff, the facility failed to maintain a current, dated, written list of all children?s allergies, sensitivities and dietary restrictions that is kept confidential in each room or area where children are present. Evidence: The list posted in the infant class was dated: 11/22/22 and not current, per staff. The list in the toddler and two year old classrooms was not posted in a confidential manner.

Description: Based on review of seven child records, it was determined that not all records contained all required information. Evidence: the record for child #7, with a diagnosed food allergy, did not contain a written care plan, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction

Description: Based on review of the written attendance record, observation, and an interview with staff, it was determined that the facility did not maintain a written record of daily attendance, for each group of children, that documents the arrival and departure of children in care as it occurs. Evidence: 1) In the infant classroom, there were seven children listed on the attendance record, eight children observed in the classroom, and one additional infant observed in the director?s office. The school age child in the director?s office was not listed on an attendance sheet.

Description: Based on observation, not all hazardous substances were kept in a locked place, using a safe locking method that prevents access by children. Evidence: there was a bottle of sanitizing spray sitting in a sink by the changing table in the two's classroom.

Description: Based on observation, the facility failed to ensure that all restrooms were equipped as required. Evidence: The bathrooms utilized by the children in the two's and preschool classes did not contain toilet paper that was within reach of children. There was no toilet paper in the school age bathroom within reach of the children and no paper towels in the school age bathroom/sink area.

Description: Based on observation, and a review of documentation, the facility failed to maintain the staff-to-child ratio of one staff for four children age birth to 16 months. Evidence: in the toddler classroom there were 14 children with 2 staff members. The youngest child in the group was 15 months old.

Standard #: 8VAC20-780-350-Q
Description: Based on observation, and an interview with staff, the facility casually reassigned an infant from their regular classroom disrupting their schedule and attachment to their staff members and group. Evidence: 1) infant child #5, age two months, was observed in the director's office sitting in a bouncy chair, in close proximity to a school age child who had been separated from their group because they were sick. 2) There were two administrative staff members sitting in the front office area. 3) Staff acknowledged that there were eight infants already present in the infant classroom with two staff when a ninth infant arrived, but they didn?t go into the class because they had work to do in the office, so when the ninth infant arrived, staff took child #5 out of the infant class and into the office.

Standard #: 8VAC20-780-500-B
Description: Based on observation, and an interview with staff, the facility failed to follow all diapering procedures, as required, including using a nonabsorbent surface for changing diapers and disposing of diapers in a leakproof storage system. Evidence: There was no changing table in the early preschool classroom and staff stated that children under three are sometimes changed standing up in the bathroom. Diapers were observed in a trash can in the two's bathroom that was not covered, therefore not leakproof.

Description: Based on observation, and a review of documentation, the facility failed to ensure that at least one staff in each classroom has current certification in CPR and first aid. Evidence: the records for staff # 1 and staff #4, who were observed alone in a classroom with children, do not contain documentation of current certification in CPR and first aid.
