An announced monitoring inspection was conducted on March 24, 2023 from 1:35 p.m. to 2:00 p.m. regarding a self-report that was received on March 20, 2023. The inspection was conducted with local Child Protective Services and the local Police Department. The inspector reviewed compliance in the areas of programs, staffing and supervision. The Licensing Inspector conducted additional interviews and an exit interview were conducted on April 6, 2023.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-410-1 Description: Based on documentation and interview, the center failed to ensure that forbidden action of roughly handling a child did not occur.
Evidence
1. Documentation provided by local police regarding an incident that occurred at the facility on 03/17/2023, ?Field Case Supplemental Report? dated 4/11/2023, stated that ?(Staff #1) admitted to sometime before lunch, she grabbed (Child #1) on his cheeks using her hand.?
Staff #1 grabbing kids and screaming at them.
3. During an interview on 4/6/2023, Staff #4 stated that they had seen Staff #1 ?being rough with children?.
4. During an interview on 4/6/2023, Staff #5 stated that they had seen Staff #1 grab a child?s collar that was standing behind Staff #1 and then pull the child around to the front of them
Staff #5 also stated that they had seen Staff #1 roughly pulling a diaper off a child.Plan of Correction: Not available online. Contact Inspector for more information.
