A renewal inspection was conducted on 04/27/2023 from 11:20 am until 2:30 pm. There were 83 children present, ranging in ages from two months to five years old, with 15 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of eight child records and eight staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Description: Based on the review of eight staff records, the program failed to ensure all employees undergo a background check prior to employment and every five years thereafter for one staff.

The date of employment for Staff #3 is 05/08/2017. The initial results of a check of the child abuse and neglect registry was dated 05/26/2017. The subsequent results were dated 07/19/2022.

Plan of Correction: After being in violation on 6/13/22, the center did a file audit to ensure that all background checks were in place. Upon discovering the late check for Staff member #3, we initiated the check on 6/16/22. On 7/19/22 we obtained those results. These results were in the file at the 4/27/23 inspection. Moving forward, there will be outlook calendar invites completed to alert all center leadership of upcoming due dates for checks. The center director will ensure this system is implemented and adhered to, so all checks are submitted far enough in advance of due dates.
