Comments:
An unannounced renewal inspection was initiated on Thursday, April 6, 2023 and concluded on Tuesday, April 11, 2023 to determine the center's compliance with licensing standards. On April 6, 2023, the inspector was on site from 9:45am to approximately 11:55am. There were a total of 39 children in care in the direct care of 10 staff members. During the inspection, the children and staff were observed participating in a variety of activities. Staff were observed having positive interactions with the children. All areas of the facility used by the children were inspected. The center is equipped with toys and supplies and items were available to the children. The required postings were reviewed and found to be in compliance. Medication is administered when required and medications were reviewed. Transportation is provided and three buses were inspected during the inspection. Five children's records, five staff records, and board officers' background checks were reviewed. Additional documentation was submitted to the inspector electronically on April 11, 2023.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on a review of five staff records and interview, the center did not ensure one staff member had a central registry finding within 30 days of employment.

Evidence: 1) The central registry finding in the record of Staff #5, employed on 09/12/22, was dated 11/03/22.

2) During interview, a member of management confirmed the results of the central registry for Staff #5 were received more than 30 days after employment. The record did not contain documentation of any further contact with the Office of Background Investigations, and the staff member had been continuously employed.

Plan of Correction: Corrected. In the future, the center will document contact with OBI when a central registry finding is not received within 30 days of employment.

Standard #: 8VAC20-780-160-A
Description: Based on a review of five staff records, the center did not ensure one staff member submitted documentation of a negative tuberculosis (TB) screening within the required timeframe.

Evidence: The TB screening in the record of Staff #5, employed on 09/12/22, was completed on 09/20/22.

Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children.

Plan of Correction: Corrected. In the future, staff will submit a negative TB screening prior to employment.

Standard #: 8VAC20-780-160-C
Description: Based on a review of five staff records, the center did not ensure one staff member had a tuberculosis (TB) screening at least every two years from the date of the first initial screening or testing.

Evidence: The most recent TB screening in the record of Staff #1, employed on 08/10/20, was completed on 12/06/22. The previous TB screening on file expired on 08/03/22.

Plan of Correction: Corrected.
