A Subsidy Health and Safety inspection was conducted on 5/01/2023 to review supplemental health and safety requirements.
The subsidy requirements were reviewed this date; allergy action plans, staff trainings, nutrition and food services, recordkeeping, hazardous substances, reporting injuries, emergency plans and transportation.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 8VAC20-790-600-F Description: Based on review of training, nine staff did not complete the department's health and safety
update course annually.
Evidence:
1. Staff #1 completed the preservice training on 2/19/2022 and had not completed the health and safety update course.
2. Staff #2 completed the last update training on 8/30/2019 and had not completed the health and safety update course this year.
3. Staff #3 completed the last update training on 3/10/2020 and had not completed the health and safety update course this year.
4. Staff #4 completed the preservice training on 12/28/2016 and had not completed the health and safety update course.
5. Staff #5 completed the preservice training on 9/26/2017 and had not completed the health and safety update course.
6. Staff #6 completed the preservice training on 2/16/2022 and had not completed the health and safety update course.
7. Staff #7 completed the preservice training on 6/02/2018 and had not completed the health and safety update course.
8. Staff #8 completed the preservice training on 9/28/2019 and had not completed the health and safety update course.
9. Staff #9 completed the preservice training on 7/02/2018 and had not completed the health and safety update course.Plan of Correction: Have 9 staff members take required updated.
