Comments:
An unannounced renewal inspection was conducted on May 1, 2023. The inspector was on site at the center from approximately 9:52am to 2:18pm. There were 94 children in attendance and a total of 14 staff supervising. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. The center?s playgrounds, medication and all classrooms were inspected today. Ten children?s records and ten employee records were reviewed during this inspection.

The information gathered during the inspection determined a violation with applicable standards or law and a violation was issued.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names, list staff by positions only.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not ensure areas and equipment of the center, were maintained in a clean, safe and operable condition.
Evidence:
The Infant B classroom had several segments of carpet that were curled up and raised, presenting a tripping hazard to children in care.

Plan of Correction: The corrective action for this issue had already begun prior to the licensing renewal visit. The carpet in our Infant Rooms is being replaced with a reliant, foam backed material with welded seams crating a solid surface which will not have any peeling issues. The quote was obtained on April 24, 2023, with installation date scheduled for the week of June 5, 2023, pending the receipt of the materials by the installer. (Quote may be made available to the licensing inspector upon request.) We have used this material in other areas of our facility with great results for safety and durability. The Facilities Manager will oversee the work to ensure completion as promised by the vendor. The Executive Director will keep the inspector apprised of the work status.

The infant areas will continue to be inspected as part of our monthly Quality Assurance Process.
