Comments:
An unannounced renewal inspection was conducted on 4/25/2023 from 10:25 am to 2:15 pm. There were 83 children in care, ages two years to six years, in seven groups under the care of 15 staff. The Licensing Inspector reviewed five children?s records, five staff records, medications, and other program records. The children were observed playing outside, doing group work, and playing inside.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on record review, the center failed to complete an out of state criminal history name check prior to the first day of employment.

EVIDENCE:
Staff 4 began employment on 2/1/2023 and the does not contain an out-of-state criminal history name check. The sworn statement for staff 4 states this person lived in a state other than Virginia within the past five years.

Plan of Correction: Out of state criminal histories will by completed ahead of first day of employment.

Standard #: 8VAC20-770-60-B
Description: Based on record review, the center failed to ensure a Sworn Statement was completed prior the first day of employment.

EVIDENCE:
Staff 4 began employment on 2/1/2023 and the Sword Statement is dated 2/8/2023.

Plan of Correction: Sworn statements will be completed ahead of first day of employment.

Standard #: 8VAC20-780-40-E
Description: Based on documentation review and observation, the licensee failed to ensure the center?s own policies and procedures were followed.

EVIDENCE:
The facility?s PROCEDURES FOR ADMINISTERING MEDICATION state, ?The Director will check the first aid station monthly to ensure that out-dated medications are disposed of.?
The LI noted that Medication 1 expired 3/5/2023 and Medication 2 expired 1/2023.

Plan of Correction: All expired medicines were disposed of. Checks will now take place on the 1st day of the month.

Standard #: 8VAC20-780-240-A
Description: Based on record review, the center failed to ensure that staff completed the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.

EVIDENCE:
Staff 1 began employment on 8/19/2022 and the Virginia Department of Education-sponsored orientation course (a.k.a. 10-hour preservice training) was completed 12/9/2022.

Plan of Correction: Preservice training will be completed within 90 days of employment.

Standard #: 8VAC20-780-240-B
Description: Based on record review, the center failed to ensure that staff completed orientation training no later than seven days of the date of assuming job responsibilities.

EVIDENCE:
1. Staff 1 started work on 8/19/2022 and there is no documentation to support that new staff orientation has been done.

2. Staff 4 started work on 2/1/2023 and there is no documentation to support that new staff orientation has been done.

Plan of Correction: Both employees were trained - but I neglected to have them signed. They will be signed at the end of training from now on.
