Areas Reviewed:
A monitoring inspection was completed from 12:10 until 2:25. There were 56 children with 16 staff present, including the cook and director. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

There were two repeat violations.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on record review and interview, the center failed to have a search of the child abuse and neglect registry which the individual has resided in the preceding five years.
Evidence:
Staff #3 (DOH 9/14/22 stated they had lived out of state in the last five years and the check had not been completed; the director verified this.

THIS IS A REPEAT VIOLATION.

Plan of Correction: The background check will be completed.

Standard #: 8VAC20-780-130-E
Description: Based on record review, the center failed to have documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
Child #3 and child #6 did not have updated immunizations on record. Updates are required of additional immunizations once every six months for children under the age of two years.

Plan of Correction: Staff will get updated immunizations from the parents.

Standard #: 8VAC20-780-330-B
Description: Based on observation and measurement, the center failed to where playground equipment is provided, resilient surfacing that complies with minimum safety standard.
Evidence:
1. On the toddler playground, there was 2-3 inches in the fall zone of the green climber and blue slide. 2. On the older child playground, there was 2-3 inches in the fall zone and around the green climber. The required depth is 6 inches.

Plan of Correction: Mulch will be ordered and spread with the required depth of 6 inches in the fall zone. The director will make sure the mulch is maintained at the required depth.

Standard #: 8VAC20-780-420-E-1
Description: Based on record review and interview, the center staff did not document all of the required information for infants.
Evidence
Staff stated tummy time was being completed but not documented; the director also verified this.

Plan of Correction: Tummy time will be documented on the on-line system.

Standard #: 8VAC20-780-550-G
Description: Based on record review and interview, the center did not complete all the required drills.
Evidence:
The director verified an evacuation drill had not been competed in January.

This is a repeat violation.

Plan of Correction: Evacuation dills will be held monthly. An additional drill will be held. The director will make sure the drills are completed.
