Technical Assistance:
Discussed activities in which children have to sit and wait for a turn need to be improved. Discussed crib spacing, changing table placement, and noticed that some cabinet doors may need to be replaced soon.

Comments:
An unannounced renewal inspection was conducted today from 9:15-10:35. There were 60 children directly supervised by 11 staff. The physical plant, 2 staff records, children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, playing outside, and eating snack. Diapering and hand-washing procedures were also observed. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance but diaper changings in the toddler class results in one staff not having direct view of all the toddlers while changing a diaper. The space in the infant's room was also discussed which included consultation about placement of equipment. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-280-H
Description: Based on observation, harmful agents are not stored in areas that are inaccessible to children.
Evidence:
1. The center has hand sanitizer for staff and parents but it was observed in two areas in which children had easy access.

Plan of Correction: Staff were talked to about keeping that substance out of reach.

Standard #: 8VAC20-780-340-A
Description: Based on interviews, record review, and observation, it was determined staff supervising children in the toddler room are not always ensuring their care and protection.
Evidence:
1. During the inspection on May 4, 2023, staff in the Toddler Class were asked how many children were in their care. Staff #1 stated there were 7 toddlers in care. When the staff was told that a count found 8 children present, the staff acknowledged she forgot another child had recently entered the room.
2. Upon observation of a bike mark on a child's arm, discussion revealed there have been approximately 10 biting incidents in the class, within the last two weeks. Also observed, was a child with a band aid on their face. The staff stated the child was scratched by another in the class and they are trying to work with the children to stop the scratching and biting.
3. Review of injury reports revealed several incidents of toddlers that had been scratched in the last two weeks and a bite injury on one child's face.

Plan of Correction: We have recently hired a new staff to help improve issues in the toddler class and will add staff in the room to help improve supervision. We will also make sure an extra staff is present in the toddler room when a staff has to go next door to change a diaper.

Description: Based on observation, interviews, and record review it was determined the center was not maintaining a written record of children's serious and minor injuries in which they were recorded and maintained on the day of occurrence.
Evidence:
1. Child in the toddler room had marks on the arm indicating a bite mark. Staff stated the child has been bit. There was not a written injury for that bite mark.
2. In the course of interviewing staff, it was revealed there had been approximately 10 bites in the last three weeks and they send home a sheet stating a bite occurred. They stated they don't typically fill out an injury report.
3. Upon review of the injury reports for the toddler room, there was only one bite injury report maintained within the last 2 weeks of injury reports.

Plan of Correction: We will be sure to document and save all injury reports. We have been sending home reports about biting but will be sure to maintain the records.
