Discussed ensuring injury reports detail the how and when and that staff understand how to record events properly.

An inspection was conducted after the office received an injury report from the center in which a 4 years old child sustained an injury on the playground. The Inspector reviewed the records, observed the resilient surfacing, and reviewed staff paperwork. During the inspection on April 25, there was a company seen placing more mulch onto the playground.
