Technical Assistance:
Technical Assistance given and Licensing Standards were reviewed regarding outdoor playground, resilient surfacing, and using staples.

Comments:
A monitoring inspection was completed May 4, 2023. Inspector arrived at 10am and departed at approximately 1:30pm. There were 54 children and 9 staff members present. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during indoor and outdoor play, group activity, lunch, diapering, and handwashing. First aid kits and emergency preparedness documents were observed. No medications are administered at the center. Transportation vans were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the Director at the exit interview.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on record review and interview, it was determined that the provider did not ensure that each child have a physical examination by or under the direction of a physician: 1. Before the child's attendance; or 2. Within 30 days after the first day of attendance.
Evidence:
1. Child #2 (date of enrollment 3/28/2022) did not have a physical examination by or under the direction of a physician.
2. Child #3 (date of enrollment 10/24/2022) did not have a physical examination by or under the direction of a physician.
3. Staff #1 confirmed that there was no physical examination documents for Child #2 and Child #3?s records.

Plan of Correction: The center responded with the following: Staff #1 will ensure to obtain a copy of the physical examinations for Child #2 and Child #3 and place them in their files at the center.

Standard #: 8VAC20-780-60-A
Description: Based on record review and interview, it was determined that the center did not ensure that they maintain and keep at the center a separate record for each child enrolled which shall contain the following information: the work phone number and place of employment of each parent who has custody; and the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
Evidence:
1. Child #1 did not have the mother?s work number listed and only had one of two designated people to call in an emergency of a parent cannot be reached.
2. Staff #1 confirmed that Child #1 did not have the mother?s work phone number listed and only had one of two designated people to call in an emergency of a parent cannot be reached.

Plan of Correction: The center responded with the following: The center will obtain the information not listed for Child #1?s file.

Standard #: 8VAC20-780-580-B
Description: Based on observation and interview it was determined that the center did not ensure that any vehicle used by the center for the transportation of children shall meet the following requirements: The vehicle shall meet the safety standards set by the Department of Motor Vehicles.
Evidence:
1. Both transportation vans used to transport children have expired Virginia State inspections.
a. Van #1 has a Virginia State inspection that expired on March 2023.
b. Van #2 has a Virginia State inspection that expired on April 2023.
2. Staff #1 confirmed that both vans that transport children to and from the center have expired Virginia State inspection tags.

Plan of Correction: The center responded with the following: Both vans will have their Virginia State inspection completed no later than the end of the month of May 2023.
