Today an unannounced Monitoring Inspection took place between the hours of 12:50pm and 2:15pm. There were 4 classrooms observed with a total of 50 children with 7 staff. The children were observed taking a nap as it was their designated rest period. A complete inspection of the physical plant, children and staff records, evacuation drill log, medication and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation during the inspection. Standard #: 22.1-289.035-B-4 Description: Based on a review of staff records, 1 out of 3 staff did not have documentation of a search being conducted of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence:
1. Staff #3 (doh: 1/17/2023) did not have documentation of a search being conducted of the child abuse and neglect registry or equivalent registry for state #1 in which the staff member has resided in during the past 5 years.Plan of Correction: We will send the background check out right away.
Standard #: 8VAC20-780-70 Description: Based on a review of staff records, 1 out of 3 staff did not have the required information in their record.
Evidence:
1. Staff #3 (doh: 1/17/2023) did not have documentation of 2 required character references prior to employment.Plan of Correction: We will get the references complete prior to employment for all future staff members.
Standard #: 8VAC20-780-350-B-1 Description: Based on observation, the infant room did not follow the required 1:4 (staff:children) ratio at all times.
Evidence:
1. From approximately 1:00pm-1:35pm (while a teacher was on lunch break), the infant room only had 2 staff in the classroom supervising at total of 9 infants.Plan of Correction: A teacher was on break and was asked to clock in and return to the classroom to maintain the required ratios. We will maintain the required ratios moving forward.
Standard #: 8VAC20-780-450-A Description: Based on observation, all cots used by children during the designated rest period did not have linens consisting of a top cover and a bottom cover.
Evidence:
1. In the Pre-K/Senior classroom, there were 4 children with no bottom cover and 2 children with no top cover.
2. In the Two's classroom, there were 2 children with no bottom cover.Plan of Correction: We will make sure all children have a top and bottom cover at rest time.
