Technical Assistance:
The provider must send documentation to the LI that the background checks have been requested no later than 10 days following this notification dated May 4, 2023. Once the background check is received by the center, the results must be forwarded to the licensing inspector.

Comments:
An unannounced monitoring inspection was conducted today from 10:30am-12:30pm. There were 63 children directly supervised by 7 staff. The physical plant, 5 staff records, 7 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in transitioning outside and playing outside. Diaper changes and handwashing were also observed. There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Description: Based on record review, a volunteer who was working in a capacity in which they were counted in ratio, did not submit to a fingerprint criminal background check.

Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. Two cabinets were observed containing spray disinfectant. The keys to the cabinets were observed to be stored in the lock.
2. A chemical sponge was observed sitting on an open sink ledge.

Description: Based on record review, the center did not follow all guidelines for administering medication.
Evidence:
1. Child 8's medication authorization does not contain the signature of a physician.
2. Child 9's medication authorization does not contain the signature of a physician and of the child's parent.
3. Child 10 and Child 11's authorization does not give "Your Child's Place" permission to administer medication. The authorization form lists that Fairfax County Public schools may administer the medication.
